What paperwork does the pharmacy require?

Onboarding and Customer Pricing Documents must be fully completed. Incomplete submissions will be returned, and providers will not be onboarded until all necessary paperwork is properly submitted.

What happens after the paperwork is submitted?

· Each provider account is assigned an internal customer support representative.

· The customer support representative will contact the provider using the details provided in the onboarding document.

· The provider must send a test prescription through their EMR system to verify the process.

· Once verified, the provider can begin sending prescriptions to the pharmacy.

How long does the signup process take?

The customer support representative will reach out within 3 business days after receiving the completed paperwork.

Shipping Information

A flat rate of $35 per order, regardless of order size.

Shipping Method

Overnight delivery via FedEx or UPS.

Packaging

Refrigerated shipping to ensure product integrity.

Reports & Review Process

· Reports will be available within 5 days after the end of each month.

Example: The January report will be available by February 5th.

· Representatives have 5 days to review and dispute the report if necessary.

Payments

Payments will be processed by the 15th of the month following the reporting period.

Example: Payment for January’s report will be made by February 15th.
